La tecnica All-on-4® nasce dalla collaborazione negli anni 90 tra Nobel Biocare®, azienda leader mondiale nella produzione di impianti dentali, e il dentista portoghese Paulo Malò, per la riabilitazione totale dei pazienti edentuli (cioè senza denti).
Essa consiste nel posizionamento di quattro impianti, due centrali dritti e due laterali inclinati di circa 30 gradi, nelle mandibole o nei mascellari superiori edentuli al fine di supportare dai 10 ai 12 denti fissi nella stessa giornata dell’intervento chirurgico.

Oggi grazie alla possibilità di programmare l’inserzione degli impianti tramite software in grado di elaborare le immagini radiografiche prodotte dalle tac, è possibile procedere all’intervento chirurgico senza più utilizzo di bisturi né punti di sutura diminuendo quindi sensibilmente il grado di discomfort e fastidio del paziente nell’immediato post operatorio.
Si è quindi potuto unire i vantaggi dell’implantologia a carico immediato (denti fissi subito) ai vantaggi della chirurgia computer guidata (chirurgia senza bisturi né suture), rendendo possibile una riabilitazione di tipo fisso in tempi molto ridotti (solitamente 4 ore) e con fastidi se non minimi, addirittura assenti.

Domande Frequenti

Il carico immediato prevede la cementazione della corona (solitamente provvisoria) il giorno stesso dell'inserimento dell'impianto. E' possibile farlo se il torque, ossia la forza d'inserimento dell'impianto nell'osso è pari o superiore a 35N*cm.

E' necessario eseguire prima degli innesti ossei prima di procedere all'inserimento dell'impianto in modo da garantire la quantità minima all'inserimento stesso. Oppure laddove possibile, inserire gli impianti inclinati (tilted), come avviene per esempio nella tecnica dell'All On Four.

Trattasi di una tecnica chirurgica che utilizza una dima per il posizionamento degli impianti. Tale dima è progettata al computer sulla base di un esame radiografico 3D (CBCT) e permette, nella maggior parte dei casi, l'inserimento dell'impianto senza necessità di lembo chirurgico e sutura.

Gli impianti sommersi sono impianti che una volta avvitati terminano a filo osso mentre quelli transmucosi sono impianti che presentano un collare lucido oltre il profilo osseo.
